Bonding a button when there is insufficient clinical crown height

Low clinical crown height, common in mixed dentition, can make button placement difficult. There are a few workarounds, and where they do not work the case needs clinical review.

What to try

  1. Use the smallest available button. The head only needs to protrude through the aligner cutout far enough to hold an elastic.
  2. Trim the aligner cutout slightly if the button head is partly obscured.
  3. In mixed dentition, consider waiting for further natural eruption before placing the button and continuing with the aligners in the meantime.

Important: If a patient wants palatal crossbite buttons removed for comfort, this needs clinical review before anything changes. Removing them compromises the crossbite correction.
