Every treating provider needs their own portal account before they can submit a case. Cases should never be submitted under a colleague's login, because the treating provider is recorded against the case and all planning communication routes to that account.

Tell your practice manager so their active cases can be reassigned and their Dental Monitoring patients transferred to whoever takes over. Patients left on a departed provider's DM account stop being supervised, and nothing in the system flags it.
